Quick answer: If you think you have a parasitic infection, the reliable path is a stool test ordered by a doctor and, if something is found, a prescription antiparasitic medication. Herbal "cleanses" are not a proven treatment, and over-the-counter or veterinary drugs taken without a diagnosis are a bad idea. Parasitic infections are uncommon in the United States but not unheard of, and the two most relevant to drinking water, giardia and cryptosporidium, are worth understanding.

What parasites are, and how common they actually are

A parasite is an organism that lives on or in a host and takes resources from it. In humans they fall into three broad groups:

  • Protozoa: single-celled organisms such as giardia and cryptosporidium. These are the ones most often linked to contaminated water.
  • Helminths: worms, including roundworms, tapeworms and flukes. Usually acquired through food, soil or contaminated water in areas without adequate sanitation.
  • Ectoparasites: lice, ticks, fleas and mites, which live on the body and can transmit other diseases.

Globally, intestinal parasites affect hundreds of millions of people, overwhelmingly in regions without safe water and sanitation. In the United States, infections are far less common, and most diagnosed cases involve international travel, occupational or animal exposure, recreational water, or a weakened immune system. That does not mean domestic cases do not happen. Giardia and cryptosporidium outbreaks are reported in the US every year, usually tied to recreational water, untreated surface water, or a failure in a water system.

It is worth being clear about one thing: online claims that most people are unknowingly carrying parasites are not supported by US surveillance data. Chronic digestive symptoms are far more often something else. A test tells you which, and that is the point of testing.

The waterborne ones: giardia and cryptosporidium

Giardia

Giardia is a protozoan parasite spread through water, food or surfaces contaminated with feces. It is one of the most commonly reported waterborne illnesses in the US, often associated with drinking untreated water from lakes and streams, swallowing water while swimming, or child care settings. Infection can cause diarrhea, gas, greasy stools, cramping and fatigue, and in some cases the symptoms are mild enough that people do not seek care. Research following outbreak patients has documented persistent fatigue and digestive symptoms in a subset of people for years after the initial giardiasis, which is one reason it is worth diagnosing rather than guessing. Cryptosporidium

Cryptosporidium spreads the same way, and it has one feature that makes it a water treatment problem: it is unusually tolerant of chlorine. That is why it turns up in swimming pool and water park outbreaks and why filtration, not disinfection alone, is the standard defense. For most healthy people an infection causes watery diarrhea and cramping that resolves on its own, but it can be serious and prolonged for people with compromised immune systems. How people actually get infected

The realistic routes, roughly in order of how often they matter in the US:

  • Swallowing untreated surface water while hiking, camping or swimming
  • Travel to areas where drinking water and food handling are less controlled
  • Undercooked pork, beef, fish or shellfish
  • Produce washed in contaminated water and eaten raw
  • Fecal-oral transmission through poor hand hygiene, child care settings or contact with infected animals
  • Insect bites, for parasites such as those causing malaria, in regions where they are endemic

Municipal tap water in the US is treated and monitored, and it is not a common source of parasitic infection. Private wells, older or damaged plumbing, boil-water notices and untreated surface water are the situations where drinking water risk becomes real.

Symptoms of parasites, and when to see a doctor

Intestinal parasite symptoms are non-specific, which is exactly why self-diagnosis does not work. Commonly reported signs include:

  • Diarrhea, loose or greasy stools, gas and bloating
  • Abdominal cramping
  • Nausea or loss of appetite
  • Unexplained weight loss
  • Fatigue

Every one of those is also caused by dozens of far more common conditions, from a viral stomach bug to irritable bowel syndrome to food intolerance. Claims that symptoms like anxiety, teeth grinding or waking at a particular hour indicate parasites are not supported by clinical evidence.

When to see a doctor

Contact a healthcare provider if you have diarrhea lasting more than a few days, blood in your stool, a fever alongside digestive symptoms, signs of dehydration, or unexplained weight loss. Seek care sooner if you have recently traveled internationally, drank untreated water, are pregnant, are immunocompromised, or the person affected is a young child or an older adult. A few parasitic infections, notably certain liver flukes and schistosomes in the regions where they are endemic, are recognized as carcinogenic, which is another reason a real diagnosis matters more than a guess.

Parasite tests for humans: how infections are diagnosed

Most intestinal parasites are microscopic, so you will usually not see anything unusual in your stool. Diagnosis is done through laboratory testing, most often an ova and parasite stool examination, sometimes with antigen tests or molecular (PCR) panels that identify specific organisms such as giardia or cryptosporidium. Because shedding is intermittent, doctors frequently ask for samples collected on more than one day.

Direct-to-consumer testing exists, but results vary in quality and a positive or negative on its own does not tell you what to do next. A test ordered and interpreted by a clinician who knows your history is the version that leads to actual treatment.

How parasitic infections are actually treated

Confirmed infections are treated with prescription antiparasitic medication chosen for the specific organism. Metronidazole and tinidazole are commonly used for giardia; nitazoxanide is used for cryptosporidium and giardia; albendazole and mebendazole are used for common intestinal worms. These are short, targeted courses, and they work far better than anything you can buy off a shelf, because they are matched to what was found.

What about parasite cleanses and detoxes?

Herbal protocols built around black walnut hull, wormwood, clove, oregano oil and similar ingredients are widely sold, and the honest summary is that there is no good clinical evidence they clear human parasitic infections. Some of these compounds show activity in laboratory settings, which is not the same as working in a person at a safe dose. Several are potent: wormwood contains thujone, which is neurotoxic at higher doses, and these products can interact with medications. The "die-off" symptoms attributed to cleanses are usually just the side effects of the product.

Eating well, staying hydrated and supporting your gut are all reasonable things to do. They are not a substitute for a diagnosis if you are actually sick.

Over the counter medicine for parasites

In the US, pinworm treatments containing pyrantel pamoate are the main over-the-counter option, and they treat one specific infection. Everything else requires a prescription for good reason. Taking ivermectin or the veterinary drug fenbendazole for an undiagnosed condition is not a recognized treatment, has real risks including drug interactions and liver injury, and is not something we would recommend to anyone. If a wellness influencer's routine is the source of a drug decision, that is the signal to talk to a doctor instead.

Preventing parasites: travel, food and water

Prevention is where most of the practical value is, and it is not complicated:

  • Do not drink untreated water from lakes, rivers or streams. Filter, boil or chemically treat it.
  • When traveling in areas with unreliable water treatment, drink bottled, boiled or properly filtered water, and skip ice made from tap water.
  • Cook pork, beef, fish and shellfish to safe internal temperatures.
  • Wash produce well, and peel it when the local water quality is uncertain.
  • Wash hands after using the bathroom, changing diapers, gardening and handling animals.
  • Do not swallow water while swimming, and stay out of the pool for two weeks after diarrhea.

The bottom line

Parasitic infections are a real medical condition with a real diagnostic pathway and real treatments. They are also uncommon in the US, and the internet dramatically overstates how many people are quietly carrying them. If you have persistent digestive symptoms, get tested rather than starting a cleanse.
